RN Clinical Team Leader— Charlestown Office

Full-time benefited position Under the
supervision of the Patient Care Manager,
the Team Leader instructs and supervises
nursing & homemaker/home health aide
staff to insure the provision of quality
service. Oversees & coordinates agency
therapeutic services and participates in
the case management decision process.
Develops and fosters relationships with
referral sources and the community.
Assists in the development and implementation of staff education and orientation.

NH RN license required.
Home care experience a must.
